Document Delivery Request Form
Each order is assessed the following charges:
- a per citation service charge of $10.
- a per page charge of $0.75 for USPS mail delivery or
- a per page charge of $0.50 for electronic (fax or e-mail) delivery
- an 8% sales tax (on in-state requests only)
- and any applicable Copyright Clearance Center charges.
In addition to the above charges, the following surcharges may apply:
- a $5. per citation fee if the source document is in microform format
- a $20. per citation special handling fee if the citation or contact information is incorrect or incomplete.
Expedited service (3 hour turnaround, available weekdays between 8am and 4pm) is available
for an additional per citation charge of $25.
Your invoice will be included with the document.
